                                                    Case Manager – Health and Human Services

Our client is a municipal department in Milwaukee County. Focused on residential construction and restoration projects, this employer is seeking a qualified, detail oriented professional to join the Resident Services team.

The Case Manager will be responsible for providing case management services to resident families. This includes assessing individual needs and preparing plans, providing referrals, and arranging for transportation. This person will conduct initial and ongoing assessments of skills, qualifications, interests, and family self-sufficiency needs, as well as prepare individualized self-sufficiency plans.

Additional duties include assisting parents and children in developing an educational plan, ensuring children are attending school, connecting children to resources, and promoting parental involvement in education. In order to help the families reach self-sufficiency, the Case Manager will support efforts to acquire housing during relocation, and assisting with job readiness. This will also involve providing information to families regarding relocation moving processes, rent assistance options, and public housing options.

Other tasks involve maintaining client files and using case management databases to document activities for tracking and reporting purposes.

Qualifications for this position include:


  • A minimum of two (2) years of case management experience.
  • Knowledge of case management principles and practices; Community-based resources, programs; Applicable Federal, state, and local laws, rules, and regulations.
  • Proficiency in MS Office Suite (Excel and Word)
  • Excellent written & verbal communication skills.
