Patient Follow Up Representative
 
Our client, one of Wisconsin’s largest healthcare organizations in the Milwaukee Area, is currently interviewing candidates for a Patient Follow Up Representative to join the team.
 
This individual will assist patients and providers by supporting the coordination of care processes across regional department lines. The Patient Follow Up Representative will accept incoming calls and assist each individual appropriately based on the expressed need of the caller.

Duties for this position include documenting call encounters by using on-line information systems during the call, completing all necessary forms, and verifying and updating the database with patient and physician information. This individual will also monitor and maintain department supply inventories. Other tasks that this individual will perform are assisting in outbound referral follow-up calls to facilities, physicians, and clinical staff on all referrals and transfers.
 
Qualifications include:

  • High school graduate/equivalent
  • 1 year clerical experience (marketing, healthcare, communications, teleservice or telemarketing)
  • Proficient with Microsoft Word and/or Excel
  • Excellent verbal communication skills to effectively interact with a diverse population 
  • Demonstrate prioritization and organizational skills
  • Must have a self-confident, positive, sales-oriented personality
